2026年5月19日,上海交通大学医学院附属瑞金医院官方发布了一例输入性Q热感染病例。患者为长期在泰国农村地区工作的44岁男性,因不明原因高热不退,在当地治疗无效后紧急回国就诊。经外周血宏基因组二代测序(mNGS)检测,最终确诊为贝纳柯克斯体(Coxiella burnetii) 感染,即俗称的Q热立克次体感染。给予米诺环素抗感染治疗后,患者体温次日即明显下降,症状逐渐缓解。
贝纳柯克斯体具有极强的环境抵抗力、极低的感染剂量和多样化的传播途径,是全球分布最广的人畜共患病原体之一。尽管Q热在临床上已有治疗药物,但在基础研究领域,其胞内寄生机制、免疫逃逸策略以及慢性感染的分子基础仍存在诸多未解之谜。

传统上,贝纳柯克斯体因形态学特征与立克次体相似而被归入立克次体目。然而,现代分子生物学研究表明,其基因组特征与γ-变形菌纲(Gammaproteobacteria)更为接近,现被正式分类为:变形菌门、γ-变形菌纲、军团菌目、柯克斯体科、柯克斯体属,是该属目前确认的物种。这一分类学调整对理解其进化关系和生物学特性具有重要意义。
贝纳柯克斯体最显著的生物学特征是其双相发育周期,在宿主细胞内交替存在两种形态和功能截然不同的细胞类型:
这种双相发育策略使贝纳柯克斯体既能在宿主细胞内高效复制,又能在恶劣环境中长期存活,为其通过气溶胶传播提供了生物学基础。
贝纳柯克斯体是专性胞内寄生菌,主要感染单核-巨噬细胞系统。与大多数胞内菌不同,它不仅能抵抗吞噬溶酶体的杀菌作用,反而能在pH 4.5-5.0的酸性吞噬溶酶体环境中大量增殖。它通过改造宿主细胞的膜系统,形成一个独特的寄生空泡(PV),该空泡具有溶酶体的特征,但被贝纳柯克斯体劫持用于自身复制。
贝纳柯克斯体拥有一个完整的Dot/Icm型IVB分泌系统,这是其最关键的毒力因子。该系统能够将超过100种效应蛋白分泌到宿主细胞内,调控宿主细胞的多种生理过程,包括:

贝纳柯克斯体存在I相和II相两种抗原变异型:
这种相变异在血清学诊断中具有重要意义:急性Q热患者主要产生针对II相抗原的抗体,而慢性Q热患者则以高滴度的I相抗体为特征。

约1-5%的急性Q热患者会发展为慢性感染,主要表现为心内膜炎和血管感染,治疗周期长达18-24个月,若不及时规范治疗,死亡率高达10-25%。慢性感染的发生机制目前尚不完全清楚,可能与以下因素有关:

目前,全球仅有澳大利亚生产的Q-Vax®疫苗上市,该疫苗是由灭活的I相贝纳柯克斯体制备而成,保护率可达95%以上。但由于其可能引起严重的局部反应和全身反应,且不能用于已感染人群,其应用受到一定限制。
